Cause
Duplicate workflow activities
Resolution
- Navigate to Workflow > Workflow Versions
- Locate and open the record of the workflow you have checked out
- Add the related list "Activities"
- Find and delete your offending or duplicated record
- Close and re-open your workflow in the editor
