Incident records created in some other table that extends task

The "Task type" (sys_class_name) of the record was changed.
Most probable cause of the issue is that the "Task Type" of the record was changed by the user after the record was created.
The cause can be verified by going to the "Audit History" of the record. (Form menu -> History -> List)
To avoid the issue, ACLs can be created for the "Task Type" field that restricts the write access to that field.
