The issue is caused due to a business rule 

Resolution


The way the priority field is populated has been customized and it is being populated from a custom business rule.

The priority rule is populated based on the values of the other fields.

There was an error in the condition of the business rule and so the business rule condition did not satisfy and therefore the Priority value was not set.
