Additional radio button showing in Service catalog item view
Procedure
When you click on 'Try it' for the catalog item, you see an additional choice other than the ones configured in Question Choices. This happens when you have 'Do not select the first choice' selected and have a default value set other than the configured choices.
In order to fix the issue, remove the default value set.
