Symptoms
Change Tasks created via template aren't linking to Change request
Cause
Misconfiguration on change_task template where the field change_request sets to empty
Resolution
There should not be any field which sets the value empty on template. So after setting the change_request field correctly on template, all the change_request created via template links to the change request.
