Adding content to the Introduction field is not saving

Duplicate Introduction and End Note fields on the form
This was caused because there are multiple Introduction and End Note fields on the survey form. This is causing the issue that you are seeing. We suggested to remove the "Introduction & End Notes" field from this form and this should work.
With the introduction of Kingston, these fields were moved to their own section called "Introduction & End Notes". Customization to the form could have caused this change to be skipped on upgrade
