To fix this specific issue:
- Install the com.snc.assessment_core (Assessment) plugin on the affected instance
- Once the plugin has been installed, go to the specific Survey in question, and you will see a related list stating: "Migrate to assessment"
- Click on "Migrate to assessment" and wait for the procedure to complete
- Once the survey has successfully migrated, you should clear the cache (with cache.do)
- Once you log in, the survey responses, should be located on the "Survey Responses" on the legacy survey module.
