The conditions in the Trigger Condition were not being met

Resolution

Noticed that when creating incidents, the incorrect choices were being made that did not match the conditions defined on the Trigger Condition. Once creating and resolving an incident based on those conditions, it was found that records were being created in the asmt_assessment_instance table
