The assessment_take2 UI page was customized.
The assessment_take2 UI page was modified. service-now.com/sys_ui_page.do?sys_id=012918babfb001007a6d257b3f073996&sysparm_view=
Revert the page to the latest version OOB and notice that the error will be fixed.
We recommend that comparing the OOB UI page with the customized UI page and make the necessary changes or keep the OOB page in place. This will avoid future issues.
