Steps to Reproduce
1. Log in to the instance using Internet Explorer 9 or 10.
2. Go to an active change_request record.
3. Edit the Short description field.
4. Click on the Calendar icon next to one of the Date/Time fields. The dialog box is triggered.
If you are able to upgrade, review the Fixed In field to determine the versions that have a permanent fix to this issue.
